Noncompliance Area: Record for Child #1 showed an enrollment date of 08/22/2024, but no initial health assessment was observed to be on file within the 60-day interval. The health assessment on file is dated 01/14/2025. Corrected at time of inspection.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. A child's record shall contain initial and subsequent health reports.

Noncompliance Area: Records for Staff #1 were observed to have current clearances, but the State Police Clearance and NSOR were obtained after the required 60-month interval. Corrected at time of inspection.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: Staff discussed feeding schedules of infants with parents. A written feeding schedule was not obtained for infants from the parents.

Correction Required: A written statement giving the formula and feeding schedule for an infant shall be obtained from the parent.

Noncompliance Area: The facility did not maintain and accident file and accident reports were not in the child's file. Parents were notified through the computer program but hard copies were not maintained at the facility.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Description: No harsh language

Noncompliance Area: Staff person #1 yelled loud and harshly at a school age child. Staff #1 yelled loud and aggressively. A child with some anxiety issues was told by one staff, "I have anxiety too, but don't act that way.

Correction Required: A facility person may not use harsh, demeaning or abusive language in the presence of children.

Noncompliance Area: Observed the toilet seat in the boys bathroom in the SACC room to be loose and the toilet was leaking.

Noncompliance Area: Observed the mats in the older toddler room not to have 2 feet of space on 3 sides while the children were napping.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: Observed outlet covers to be missing from the PS and PK-counts room .

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Description: General Health and Safety

Noncompliance Area: Child was able to pull a hot cup of coffee off a table resulting in the child getting hot liquid on themselves. The child had 3 small blisters on their stomach.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: The file of facility person #1, hired 04/21/2022, does not contain completed out of state child abuse, or criminal history information required under the CPSL and does not contain a copy of a request for out of state child abuse history clearance or a copy of a request for criminal history clearance required for provisional hiring under the CPSL.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # 1may not work in a child care position at the facility.

Description: No peeling paint or plaster

Noncompliance Area: Observed the paint to be chipped and peeling near the entrance door in the SACC room.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: Observed small items less than inch to be present in the 203 year old room.

Correction Required: Pea gravel and other materials with a diameter of less than 1 inch may not be used in spaces where infants or toddlers receive care.

Noncompliance Area: Observed the mats not to be marked for a specific child's use in the 203 year old room.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Observed medication in the Owls room not to be locked. Medication was accessible to children using the handwashing sink.

Correction Required: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.

Noncompliance Area: Observed open food requiring refrigeration not to be refrigerated in the 2-3 year old room.

Correction Required: Food handling practices shall conform to the requirements of the Department of Health or the Department of Environmental Resources or its delegate agency where the child care facility is located.

Noncompliance Area: Observed the canopy and wooden structure of the sandbox located in the outdoor space to be in disrepair.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Children's files were reviewed, child #1 did not have an initial health assessment on file.

Correction Required: The initial health report for an older toddler or preschool child shall be dated no more than 1 year prior to the first day of attendance at the facility.

Noncompliance Area: Staff files were reviewed, the file for staff person #1 did not contain a health assessment conducted including TB test with results within the last 12 month prior to date of hire.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: Observed the ceiling of the gymnasium to be damaged, with plaster missing.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed chipped and peeling paint to be present in the following areas, 1. In the preschool room next to the office on the wall nearest the office, 2. In the one year old room on the windowsill.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: Observed plastic bags in the boys bathroom to be down and accessible to children.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: Staff admitted to having Tylenol in her purse which was accessible to children in the preschool room.

Correction Required: Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.

Noncompliance Area: Staff files were reviewed, staff person #2 did not have an updated health assessment on file.

Noncompliance Area: Staff files were reviewed, staff person #1 did not receive annual fire safety training.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: 1. Observed Clorox wipes, in the boys bathroom, to be accessible to children. 2. Observed a small bottle of oils, with the label of 'keep out of reach of children' to be accessible to children in the pre-k room.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: observed the air vents in the school age room to be dirty.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Observed chipped or damaged paint to be present in the following areas, school age large motor area, the pre-school room near the window, and the back wall of the toddler room.
