Description: The Warren Street Center for Children and Families provides free and low-cost day care and academic enrichment services to about 120 children a year between the ages of 2 months and 12 years.
The Warren Street Center is licensed by the New York City Department of Health and Mental Hygiene (DOHMH).

|Brief Description: The registrant must prepare a health care plan on forms furnished by the Office. Such plan must protect and promote the health of children. The health care plan must be on site, followed by all staff and available upon demand by a parent or the Office. In those instances in which the program will administer medications, the health care plan must also be approved by the program's health care consultant unless the only medications to be administered are:|
|Brief Description: Each director and each staff person working an average of twenty (20) or more hours a week must complete a minimum of thirty (30) hours of training every two years. Such training requirements shall also apply to any volunteer in such program who has the potential for regular and substantial contact with children.|
|Brief Description: documentation from an inspector from the New York State Department of Labor, or an insurance company licensed to write boiler insurance in New York State, showing that all steam or hot water boilers have been inspected and approved during the current license period in accordance with the requirements of the New York State Department of Labor. For all other fuel burning heating systems and equipment, and boilers not subject to the New York State Department of Labor requirements, documentation of service by a heating contractor performed within the 12 months preceding the date of application for renewal.|
|Brief Description: a copy of a certificate of insurance from an insurance company showing the intent to provide general liability insurance to the program upon registration and a copy of the insurance policy issued after licensure;|
|Brief Description: Staff and volunteers must each submit a medical statement on forms furnished by the Office or an approved equivalent from a health care provider:|

|Brief Description: The initial medical statement for staff and volunteers must include the results of a Mantoux tuberculin test or other federally approved tuberculin test performed within the 12 months preceding the date of the application.|
|Brief Description: Every director, teacher, and volunteer with the potential for regular and substantial contact with children in care must complete Office-approved training that complies with the federal minimum health and safety pre-service or three-month orientation period training requirements.|
|Brief Description: a sworn statement by the applicant indicating whether, to the best of the applicant's knowledge, such applicant has ever been convicted of a misdemeanor or felony in New York State or any other jurisdiction and fingerprint images as required to comply with the requirements of 413.4 of this Title; and|
|Brief Description: the information necessary to determine whether the applicant is the subject of an indicated report of child abuse and maltreatment as required by section 414.10(b)(i) of this Part.|
