Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The file of child #1 lacked an emergency contact form with the policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: Observed Peroxide and Family wellness Triple Antibiotic ointment not labeled for the specific use of a child.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: The file of staff #1 lacked documentation of completing the annual minimum of 6 clock hours of child care training.

Correction Required: A staff person shall obtain an annual minimum of 6 clock hours of child care training. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: Observed Aquaphor labeled with "Keep out of reach of children" stored in a bin labeled for child #2 which was located on a lower shelf making it accessible to children in care.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: Observed Infant's Tylenol Pain and Fever with an expiration date of 8/2018.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: The file of staff #1 lacked a current health assessment. The most recent assessment on file was dated 11-15-16.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Observed a red block with splintering edges located on the book shelf at the entrance. Observed on that same book shelf the third shelf on the left side had an exposed knot of wood with sharp, rough edges.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The file of child #1 with a start date of 10-6-15, lacked an initial health assessment.

Correction Required: The initial health report for an older toddler or preschool child shall be dated no more than 1 year prior to the first day of attendance at the facility.

Noncompliance Area: Observed expired creams labeled for specific children: Moisture Barrier Antifungal cream with an expiration date of 5/2017 and Eucerin with an expiration date of 8/2017.

Correction Required: Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Noncompliance Area: At 12:05 p.m. upon entering the back porch and yard where the children in care were eating their lunch, there were 17 children. Present were 4 infants, 3 older toddlers, 6 pre-school aged children and 4 school-age children with 4 staff. Staff #1 identified 3 infants, Staff #2 identified 6 pre-school aged children, staff #3 identified 4 younger and older school-age children. When certification staff asked staff #4 to identify the children in her group, staff #4 stated she was only a floater during lunch to prep and run back and forth. This left 4 children: 3 older toddlers and 1 infant not identified by any staff member. These 4 children were located in high chairs on the porch.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: Observed 15 unrelated children in care, the total capacity for a group child care home is 12 unrelated children, unless they only serve older school-age children.

Correction Required: No more than 12 children unrelated to the operator may simultaneously receive care except in a facility serving older school-age children. In a facility serving only older school-age children, a certificate of compliance may be issued for up to 15 children unrelated to the operator.
