Noncompliance Area: Based on interviews with two people from an outside agency during complaint investigation, witness #1 reported to have seen staff #2 forcefully pull a child by the arm. Witness #1 did not know the name of the child. This incident occurred on March 25, 2025. Witness #2 reported to have seen staff #1 grabbing a child by the face and yelled directly at the child. Witness #2 did not know the name of this child. Witness #2 did not recall the exact date in which this was observed.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child. A facility person may not use harsh, demeaning or abusive language in the presence of children. Tiered LIS 1. Children must be supervised at all times. A facility may not use harsh, demeaning, or abusive language in the presence of children. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff to receive a minimum of two hours of in person training regarding caring for children with difficult behaviors. The legal entity must receive DHS approval for the training. The operator shall provide a date for when this training will be completed.

We recognize the importance of maintaining the highest standards of supervision and professionalism in our classrooms at all times. To reinforce this commitment, the following steps will be taken: Children will continue to be supervised at all times. Staff will maintain a positive, respectful tone in all communication and will refrain from interactions that could be perceived as harsh, demeaning, or inappropriate in the presence of children. The Director of Children & Teens and Progressive Center Director, with support from our STARS Coach, will coordinate a two-hour, in-person training for teaching staff. This training will focus on supportive strategies for working with children who exhibit challenging behaviors. The content will be reviewed and approved by the state in advance, and the training will be completed no later than September 20, 2025, to allow adequate time for planning and approval.
