Description: The School Age Child Care program is sponsored by the Town of Smithtown. This self-sustaining school based program, designed for working families, is currently offered at the following schools: Smithtown School District: St. James, Smithtown, Accompsett, Dogwood, Branch Brook, Mt. Pleasant, Mills Pond, Tackan and Nesconset Elementary Schools. Kings Park School District: Parkview and Ft. Salonga Elementary Schools

|Brief Description: a completed application for renewal, including required attestations, on forms furnished by the Office, or approved equivalents. Such application and attestations must include an agreement by the applicant to operate the school-age child care program in conformity with applicable laws and regulations;|
|Brief Description: The licensee or registrant shall furnish to the applicant, a fingerprint imaging application form and a description of how the completed fingerprint images will be used. If the licensee or registrant obtains an applicant's completed fingerprints images before the images were submitted to the Office, the program shall promptly transmit such completed fingerprint images to the Office.|

|Brief Description: Licensed authorized prescriber's name, telephone number, and signature;|
|Brief Description: The program must obtain an individual health plan for any child identified as a child with special health care needs, prior to the child being in attendance at the program.|
|Brief Description: must notify the Office immediately in writing when there is any change in director;|
|Brief Description: must submit to the Office within 15 days of the written notification of the change in director the name of any new director and the supporting documentation needed to complete the approval process including:|
|Brief Description: Upon termination or resignation of the director, an acting director must be immediately named by the registrant. The appointment of the acting director must be shared with the Office by the next day of business. A permanent director must be on-site within ninety days.|
|Brief Description: Mid-point requirements for four-year registration holders. At the two-year calendar date in a four year registration cycle, a program must be in compliance with the following mid-point requirements and be able to show proof of compliance to the Office when requested, as follows:|
|Brief Description: proof of compliance with the training requirements of section 414.14;|
|Brief Description: Whenever change(s), addition(s) or expansion(s) are proposed which will affect, or reasonably may be expected to affect, those portions of the building designated for the care of children or for their egress in case of an emergency, the program must receive written approval from the Office prior to initiating such change(s), addition(s), or expansion(s).|
|Brief Description: Evacuation drills must be conducted at least monthly during various hours of operation of the school age child care program.|
|Brief Description: Staff and volunteers must ensure that children thoroughly wash their hands or assist children with thoroughly washing their hands with soap and running water when they are dirty, after toileting, before and after food handling or eating, after handling pets or other animals, after contact with any bodily secretion or fluid, and after coming in from outdoors.|
|Brief Description: Each school age child care program must operate in compliance with the regulations of the Office and all other applicable laws and regulations.|
