LS has been working with Owner and Director to get new equipment for the playground. Owner has found safe and appropriate outdoor flooring to place on pavement in playground area. Flooring needs to be installed July 1, 2022. Center needs to make it a priority to provide this playground equipment so the children can play outside during the summer.

Findings: The toddler playground has a thin layer of sand on top of a parking lot, making it slippery. The preschool playground has large sticks that the teacher reports having to throw back in the woods so children do not hit each other. LS suggested that one staff member go out each morning and clean up playground before children come out to play.

Findings: The large amount of accident reports documented made the LS aware that adequate supervision was not being practiced. LS discussed with Owner the importance of supervision, preventing injuries and being proactive from now on to prevent any more injuries. Owner will conduct a staff meeting and teach all staff members how to station themselves in the classroom and on the playground in order to provide adequate supervision at all times.

Findings: While conducting interviews with the preschool teachers, one teacher admitted to staying at the top of the hill because she was not able to walk down the hill which prevents her from providing immediate assistance when necessary. Regarding the complaint, neither teacher saw or knew about the injury in question. Both teachers said the child never cried or came to them about the injury.
