The children shall be supervised directly by a staff member at all times, including during outdoor activities, rest and sleep, and walking through hallways. Toileting procedures shall be supervised by a staff member as appropriate for the ages and developmental needs of the children.

Violation Observed: Based on information, ensure that adequate supervision is maintained at all times throughout the center. A named staff left the children unsupervised while answering the door to get a food delivery.

There shall be a minimum of two staff members accompanying children on any field trip, outing, or special event involving children away from the center, even when the appropriate staff/child ratios allow fewer than two staff members.

Violation Observed: Based on information, ensure that minimum staffing levels are maintained as required. There was 1 staff present at the center when 11 children were present.

Topics of orientation training shall include understanding center operations, policies and procedures, as specified in N.J.A.C. 3A:52-4.5(b)3.

2021-06-07 2021-05-04 3A:52-4.9(a) Special requirements to prevent child abuse and neglect
The sponsor, sponsor representative, director, or any staff member shall verbally notify the State Central Registry Hotline (1-877 NJ ABUSE/1-877-652-2873) immediately whenever there is reasonable cause to believe that a child has been subjected to abuse or neglect by a staff member, or any other adult, pursuant to N.J.S.A. 9:6-8.9, 8.10, 8.13 and 8.14.

The sponsor or sponsor representative shall secure comprehensive general liability insurance coverage for the center and shall maintain on file a copy of the insurance policy or documentation of current insurance coverage.

Violation Observed: Ensure the center has commercial comprehensive general liability insurance coverage. The insurance only indicates coverage for worker's compensation and employer liability.
2021-03-29 2021-05-04 3A:52-6.4(i) Rest and sleep requirements for early childhood programs
Children shall not use pacifiers with straps or other types of attachment devices.

Violation Observed: Ensure the children in room 2 are not permitted to use pacifier straps. The strap was removed during the inspection.
2021-03-29 2021-06-07 3A:52-6.3(b)(4)(iv)(2) Food and nutrition
Mealtime and snack requirements for centers serving children 18 months of age or older and those children younger than 18 months of age who are developmentally ready to eat regular meals and snacks are as follows: centers that provide meals shall ensure that lunch and dinner for each child include the following fruits or vegetables.

Violation Observed: Ensure the children's lunch, when provided by the center, includes fruit or vegetable. The center served the children pizza only.
2021-03-29 3A:52-4.8(d)(1) Orientation training
In lieu of the child care staff development specified in N.J.A.C. 3A:52-4.8(b), the director, head teacher(s), group teacher(s), and program supervisor(s) shall each complete 20 hours of staff development each year. Recommended topics of training for these staff include educational and physical activity.

2021-03-29 2021-05-04 3A:52-7.11(a)(2) Information to parents regarding the management of communicable diseases
Each center shall develop a written policy on the management of communicable diseases. This policy shall include the following: the list of diseases for which a child will not be readmitted to the center unless accompanied by a statement from the child's health care provider, as specified in N.J.A.C. 3A:52-7.1(d).

Violation Observed: Ensure the current communicable disease list is on site and provided to the parents. The communicable disease policy is outdated and contains a disease that is no longer considered a communicable disease by the CDC.

Indoor maintenance and sanitation requirements are as follows: floors, carpeting, walls, window coverings, ceilings, and other surfaces shall be kept clean and in good repair.

2021-03-29 3A:52-4.8(c) Orientation training
The center shall ensure that all staff members who work at the center complete 12 hours of continuing staff development each year.

Violation Observed: Ensure the non credentialed staff receive 12 hours of continuing staff development training.
2021-03-29 3A:52-4.5(e)(8) Staff responsibilities
For early childhood programs all staff that work with children under 2 1/2 years of age shall have training specific to this age group, which may include, but is not limited to, the New Jersey Infant/Toddler Credential administered through Professional Impact NJ, the Infant-Toddler Child Development Associate (CDA) Credential, or other trainings specifically related to the care of infants and toddlers. Each employee shall initiate this training within 90 days and complete it within one year.

Violation Observed: Ensure the staff working with the three children under the age of 2.5 have age related training.
