Noncompliance Area: On 1/10/23 at 9:20 AM, Staff Persons #3 and #10 were observed supervising a mixed group of 14 children (12 younger toddlers and two older toddlers) in the Young Toddler Room. Staff Person #9, who was also assigned to the room was preparing and serving breakfast at this time. Based on sign-in sheets and conversation with staff, the room was over ratio from 9:00 - 9:20 AM.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level).

Noncompliance Area: Two toilet stalls in the Preschool Bathroom lacked handwashing signs.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: Based on conversations with staff, it was confirmed that Child #1 fell and chipped her teeth causing her lip to bleed on July 3, 2023 and an injury report was given to the parent. This accident/injury was not documented in the accident file or child's file.

Correction Required: A child's record shall contain reports of accidents, injuries and illnesses involving a child in care at the facility. The original report shall be given to the parent on the day of the incident. The second copy of the report shall be retained at the facility in an accident file. The third copy of the report shall be retained at the facility in the child's file.

Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.
