Noncompliance Area: A pack and play in the Infant Room was not labeled for the child whom was sleeping in it.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The files for Child #1 and #4 contained emergency contact forms which lacked the addresses for release persons.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: The files for Child #2, age 2, #3, age 5, #4, age 2 and #5, age 3, contained documentation of immunization histories which lacked current influenza vaccinations.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The file for Staff #1 contained 5 of the 6 required hours of annual child care training for her anniversary year.

Correction Required: A staff person shall obtain an annual minimum of 6 clock hours of child care training.A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: The first-aid kit in the School-Age Room was observed to lack sterile gauze pads.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: Peeling paint and plaster were observed below the front bulletin board of the School-Age Room.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: Foam blocks in the Toddler Room were observed to contain bite marks and pieces missing.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The left drawer face on the microwave cart in the Pre-School Room was observed to be pulling off, exposing two nails.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The file for Child #2 contained an emergency contact form which lacked the address for a release person.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: The file for Child #1 contained consecutive health reports dated more than 12 months apart.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: The file for Staff #1 contained consecutive health assessments dated more than 24 months apart.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: The file for Staff #2 contained an FBI clearance dated after the first day of employmnet in a child care position. The file for Staff #2 lacked documentation of a valid fingerprint receipt.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services).A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: Sunscreen labeled "Keep out of the Reach of Children" was observed to be accessible to children in the cubbies of the Pre-School Room. A Natural Magic odor absorber gel labeled "Keep out of the Reach of Children" was observed on the floor in the back left corner of the Gross Motor Room, accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: The middle window in the Pre-School Room, which is on the second floor, was observed to be able to be opened more than six inches.

Correction Required: Windows or doors above the ground floor that open directly to the outdoors and are accessible to children shall be constructed, modified or adapted to limit the opening to 6 or fewer inches.

Noncompliance Area: Peeling paint was observed in the back left corner of the School-Age Room. Peeling paint and plaster was observed in the back right corner of the Gross Motor Room. Peeling paint was observed by the Robert Smith Room sign on the left wall of the Gross Motor Room.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
