Violation Observed: Based on the information, ensure that adequate supervision is maintained at all times throughout the center, including during transitions. A named child was left unsupervised for approximately 45 minutes on the playground when the class transitioned back to the classroom.

The center shall develop and implement an Office of Licensing-approved method to keep track of the location and ensure the safety of all children at all times when under the center’s supervision, including the transfer of supervision from and to parents during arrival and departure and the utilization of off-site locations, including playgrounds and field trips.

Violation Observed: Ensure staff know the number of children in their care at all times. Staff stated they had 11 children when 10 were present. | 1/3/2020: Staff in room 4 did not know how many children were present and had to count.
