During the walk through of the classrooms it was noted that in the space for children three through four years of age the storage closet used for cots was left opened while staff were putting out cots. Inside the closet there were several items like Clorox, Dreft laundry detergent, Lysol in an aerosol can and a can of disinfectant in an aerosol can that were considered hazardous. In the staff restroom located in the hallway there was a can of Lysol in an aerosol can stored in an unlocked cabinet.

Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. During the walk through of the classrooms it was noted that in the classroom for children ages one through two years of age there was one staff member with seven children ages one through two years. The other teacher had stepped out of the classroom briefly leaving the other teacher out of ratio.

Children were not adequately supervised at all times. On 02-07-2023 two staff members left the playground unknowingly at the same time leaving the classroom without an adult on their side of the playground for one (1) minute and fifteen (15) seconds. At 10:33:37 am, one of the teachers left the playground by way of the end entrance door off the playground into the building to go to the bathroom. At the exact same time, the other teacher was called to the door on the other end of the playground and asked to go help with coverage in another room. The children were on the playground and noticed at 10:34:02 that they were alone on the playground. The children went to the fence of the adjoining playground (1 and 2 Year Old Playground) and spoke to the teacher of the playground. The first teacher arrived back on the playground from the bathroom at 10:35:12.
