The center shall take any steps required by the Office of Licensing to correct conditions in the building or center that may endanger the health, safety, and well-being of the children served.

Violation Observed: On the day of the inspection a sick child was dismissed from the elementary school day into the program by the district school nurse. This sick child's parents had been called earlier during the school day, but hadn't picked her up as of the end of the day. This child was sat in the gym in a chair by the nurse in an attempt to keep her isolated. The child was sitting next to several children and was not isolated. Another similar incident happened the previous Friday when a sick child was sent to after care and not home and then threw up 3 times during the after care program. Ensure children that become sick during the school day are sent home and not to after care. just because the child is enrolled in after care does not mean that the program can accept sick children.

The center shall develop and implement an Office of Licensing-approved method to keep track of the location and ensure the safety of all children at all times when under the center’s supervision, including the transfer of supervision from and to parents during arrival and departure and the utilization of off-site locations, including playgrounds and field trips.

Violation Observed: On the day of the inspection staff were observed talking in a group and not actively supervising the children why the were running around, climbing on the multiple climbing structures and when they were on the courtyard playing basketball. Staff must be actively supervising children at all times.

The staff member(s) specified in N.J.A.C. 3A:52-4.5 who are responsible for developing and overseeing the implementation of the center's daily activities shall ensure that time frames for each activity are geared to the age and developmental level of each child served and are flexible enough to accommodate spontaneous occurrences or children's suggestions and inquiries.

Violation Observed: On the day of the inspection 40 children sat for over 15 minutes without anything to do or being offered an activity by staff. The children waited until the Director/program supervisor told the staff to provide an activity. Ensure that children do not have to wait for a directive from the director to be provided with an activity.

When the center applies for a new or renewal license or Certificate of Life/Safety Approval, the sponsor or sponsor representative shall submit to the Department the completed CARI consent forms for all staff members who are or will be working at the center on a regularly scheduled basis. Until the results of the CARI background check for a new staff member have been received from the Department, the center shall ensure oversight of the new staff member by another staff member, and ensure that person is not left alone to supervise a child or group of children.

Violation Observed: Ensure that CARI background checks are conducted for each staff person specific to the license and kept on file.

As a condition of securing a license or Certificate of Life/Safety Approval, the sponsor or sponsor representative shall ensure that a Criminal History Record Information (CHRI) fingerprint background check is completed for himself or herself, and for all staff members at least 18 years of age who are or will be working at the center on a regularly- scheduled basis, to determine whether any such person has been convicted of a crime, as specified in P.L. 2000, c. 77 (N.J.S.A. 30:5B-6.10 to 6.17). The sponsor or sponsor representative and each staff member shall complete the electronic fingerprinting process through the vendor authorized by the State to conduct CHRI background checks through the Division of State Police in the Department of Law and Public Safety and the Federal Bureau of Investigation.

Violation Observed: Ensure Orientation training is given on a yearly basis. Documentation shows that the staff have not been given orientation since 2017.

Handwashing requirements are as follows: the center shall ensure that children three months of age and older wash their hands with soap and running water before intake of food.

Violation Observed: On the day of the inspection children were observed being given snack and they did not wash their hands. Ensure that children wash their hands before coming to the program from another school program. Staff should not assume that they washed their hands but ensure that they do.

Space and room requirements are as follows: all space and rooms within the center to be used by children shall be inspected and approved by the Office of Licensing prior to their use. In making its determination, the Office of Licensing shall consider whether the space is too far removed, remote, or isolated from other areas of the center to be used by children. rooms or areas of the center that are not Office of Licensing-approved for use by children shall be made inaccessible to children.

Violation Observed: Center using rooms that have not been previously approved. Ensure all rooms are approved by OOL before being used by the children.

First aid requirements are as follows: at least two staff members who have current certified basic knowledge of first aid principles and cardiopulmonary resuscitation (CPR), as defined by a recognized health organization (such as the American Red Cross), shall be in the center at all times when enrolled children are present.

Violation Observed: Ensure that there are 2 staff members present with current CPR & First Aid cards at all times. There are multiple days weekly that there are not 2 certified staff members present.

Space and room requirements are as follows: at no time shall a center operate in violation of the written conditions on its license or Certificate of Life/Safety Approval including, but not limited to, capacity or age restrictions for specific classrooms.

Violation Observed: On the day of the inspection, and weekly on Wednesdays, the center is operating over its' licensed capacity of 75. There are 87 children present on Wednesdays and the center is licensed for 75 children. Reduce the number of children on Wednesdays or request an increase to the center's license.

The center shall meet the following requirements for orientation training: the center shall ensure that all newly-hired staff receive orientation training within two weeks of hire and that no individual is left alone to supervise a child or group of children until orientation training is completed.

Violation Observed: Ensure all CARIs, CHRIs, and orientation training complete before being left alone with children.

The children shall be supervised directly by a staff member at all times, including during outdoor activities, rest and sleep, and walking through hallways. Toileting procedures shall be supervised by a staff member as appropriate for the ages and developmental needs of the children.

Violation Observed: On the day of the inspection multiple times children were observed walking in the hallway alone. Ensure children are kept within line of sight at all times.

Violation Observed: On the day of the inspection there were 40 children with 2 staff members in the gym when 3 staff were needed. Ensure that ratios are maintained at all times.
