Noncompliance Area: During the inspection, the file for Child #1, enrolled 8/20/18, contained a health report dated 5/16/19 but an initial health report was not requested or on file.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: During the inspection, the legal entity did not have a current copy of the comprehensive general liability insurance policy on file at the facility.

Noncompliance Area: During the inspection, Facility Persons #1 and #2, household members present in the facility while children are in care, did not have completed child abuse, FBI fingerprint information and criminal history information required under the CPSL and did not have a copy of a request for child abuse history clearance/a copy of a request for criminal history clearance/a request for FBI fingerprint clearance/a signed disclosure statement required under the CPSL.

Correction Required: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 and #2 may not work in a child care position at the facility.

Noncompliance Area: During the inspection, protective receptacle covers were not placed in electrical outlets by the sofa and over the bathroom sink.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: During the renewal inspection on 12/27/18, disinfectant spray, disinfectant cleaner and window cleaner were accessible to children in the bathroom cabinet under the sink.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: During the renewal inspection on 12/27/18, the first aid kit did not contain tape.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: During the inspection it was determined 18 children, unrelated to the operator were in care.

Correction Required: No more than 12 children unrelated to the operator may simultaneously receive care except in a facility serving older school-age children. In a facility serving only older school-age children, a certificate of compliance may be issued for up to 15 children unrelated to the operator.
