Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: An unannounced swim visit was conducted on 7/26/2019. During the visit, two letters of reference, a health assessment and TB screening was not on site at the swimming pool for staff 1, who was the lifeguard. It was also confirmed that they were not at the facility itself.

Noncompliance Area: During an unannounced swim visit on 7/26/2019, it was found staff 1, who was the lifeguard, was 15.

Correction Required: Competence in lifeguard training is demonstrated by completion of lifeguard training, including first-aid training and cardiopulmonary resuscitation (CPR) for child and infant. When children are swimming, supervision shall include one person certified in lifeguard training, as described in 3270.31(e)(4)(iii) (relating to age and training).

Noncompliance Area: The pre k 2 room, which is being used by the school agers for the summer, did not have a school age daily schedule posted.

Correction Required: The written plan of daily activities shall be posted in the group space.

Noncompliance Area: Child 1 did not have an updated influenza shot on file.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The temperature of the hot water in the pre-k 1 and preschool room was 117 degrees.

Correction Required: Hot water pipes and other sources of heat exceeding 110 F that are accessible to children shall be equipped with protective guards or shall be insulated to prevent direct contact.

Noncompliance Area: The first aid kid in the preschool room was on the floor in the back of the preschool room, making it accessible to the children.

Correction Required: A first-aid kit must be inaccessible to children.

Noncompliance Area: In the toddler 2 room, plastic bags were accessible to the children on the changing table.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: Child 1's fee agreement was updated on 6/30/2017 and not again until 4/23/2018. Child 1's emergency contact form was updated on 6/12/2017 and not again until 2/16/2018. While they are currently up to date, they were not previously updated within a 6 month time frame.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the informationA parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: THERE WAS A CONTAINER OF SANITIZING WIPES NEXT TO THE HAND WASHING SINK IN THE SCHOOL AGE ROOM.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.
