Date Type Regulations Status
2022-06-06 Mandatory Review 13A.15.05.05A Open

2022-06-06 Mandatory Review 13A.15.05.06F Open
Findings: There was a blanket in the pack and play with the sleeping 17 month old. It was bunched up and he was sleeping on it. Provider is reminded that there should be nothing in the pack and play with the child.
2022-06-06 Mandatory Review 13A.15.08.01D(1)(a) Open
Findings: When specialist arrived the provider was up in the family room and there was a 17 month old sleeping in a bedroom downstairs off the daycare room. The provider was not in sight and sound of the child. Provider didn't have a video and sound monitor. Discussed that provider must be within sight and sound of a sleeping child under 2 years old.
2022-06-06 Mandatory Review 13A.15.08.01D(3) Open
Findings: The 17 month old was sleeping in a bedroom with the door pulled to and just open a crack. Provider is reminded that the door must be open if the child is sleeping in the bedroom and the provider must be within sight and sound so the child may have to rest in the daycare room. It would be difficult for the provider to always have to be at the bedroom door in order to maintain sight and sound of the sleeping child and maintain supervision and interaction with the other children who are awake.
2021-06-03 Full 13A.15.03.02A Corrected
Findings: PA did not have a health inventory and an immunization record. AW needs an updated shot record that shows she has had a 4th DTP shot and a Hib shot after a year of age. PP needs an updated shot record that shows she has had a 4th DTP shot and a Hib shot after a year of age.
2021-06-03 Full 13A.15.03.02B Corrected
Findings: AW, MA, and PP need evidence of having had a lead blood test.
2021-06-03 Full 13A.15.03.03A Corrected
Findings: Since the file for TG could not be located there was no evidence of having a parent agreement for that child.
2021-06-03 Full 13A.15.03.03E Corrected
Findings: Since the file for TG could not be located there was no evidence that the parents had received the consumer pamphlet.
2021-06-03 Full 13A.15.03.04A(1) Corrected
Findings: Specialist did not see an emergency card for TG. Provider may have misplaced it. Provider needs to be sure that she has an emergency card for TG.
2021-06-03 Full 13A.15.03.04C Corrected
Findings: The file for TG could not be located. The provider needs to locate the file or the paperwork will need to be recreated. Due to PA not having the parent part of the health inventory the provider did not have the child's individual needs information. Once the parent part of the health inventory is completed the provider will have that information.
2021-06-03 Full 13A.15.05.05A Corrected
Findings: The rope ladder is not secured on the bottom left. This needs to be repaired or removed.
2021-06-03 Full 13A.15.10.01H(1) Corrected
Findings: The first aid kit needs a large thick gauze pad.
2020-07-13 Mandatory Review
Findings: No Noncompliances Found
2019-06-06 Full 13A.15.05.04B(3) Corrected
Findings: The medicine cabinet latch and the sink cabinet latch in the bathroom needed adjusting to prevent access to the cabinets. This was corrected during the inspection.
2019-06-06 Full 13A.15.10.01A(3) Corrected
Findings: The provider didn't have the fire drills from August 2018 to December 2018 to show provider. She states they are on the computer and she didn't have access to that during the inspection. She needs to send specialist the record.
