Description: The outdoor play space lacked at least nine inches of energy-absorbing surfaces. The mulch, energy-absorbing material, located at the bottom of slides and under equipment of four feet or more in height, was noted to only be a few inches, the remainder of the mulch was compacted into a harder substance.

Description: The premise was not free from potential sources of harm at the time of licensing visit. Gaps were observed at the bottom of the fence area, along the West side. Nightshade was accessible in various areas in the outdoor play space, the berries accessible from the plants are poisonous. A piece of wire was observed, exposing a sharp end near the Preschool door, by the exterior meter. The gutter between the two play spaces, near the connecting gate, had a hole in it, exposing sharp edges of the metal.
