The bottle of Pedia Sure has a warning label indicating the product contains milk and soy ingredients. The staff who served the child the Pedia Sure stated she was aware of the child's allergy and what the child was typically served at meal time.

Description: Child 1 exited the classroom and the building without staff supervision, making their way onto a busy road without supervision. When the child was eventually located, the child was walking down the center stripes of Alderson Street near the intersection of Alderson Street and Schofield Avenue. When the child was located, an unknown female was stopped on the side of the road attempting to coax the child out of the center of the road. The child was without supervision for several minutes.

Description: Center guidance policies for addressing persistent inappropriate behavior include consulting with parents and also state communication with parents should be open and occur often. Contrary to the policies, staff sent Child 1 home after an incident on 7/12/2018, where center staff did not provide adequate supervision to the child. The day after the incident, the parent was told the child could not return to the center. Parents of the child had not been informed of any previous behavior concerns regarding Child 1.
