Correction Required: Staff persons shall complete professional development within 90 days of hire as listed in subsections (f)1-10. Staff persons shall complete professional development in the topic of pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.

Noncompliance Area: At time of inspection on 11/12/24, staff person #1 acknowledged that on 11/4/24, staff person #2 was holding child #1 while trying to get a bottle out of the bottle warmer. While trying to pull the bottle out of the warmer, condensation on the warmer handle/bottle splashed onto the child's ankle, causing the child to receive a 2nd degree burn from the hot steam/condensation.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110° F.

Noncompliance Area: During renewal inspection on7/30/24, the rest equipment in room 2B was not labeled with individual child names

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Description: ACIP recommended immunization record /Exemption documentation from parent/guardian

Noncompliance Area: During renewal inspection on7/30/24, the health report on file for child #1 (enrolled for more than 60 days) did not include all immunizations in accordance with the recommendation of the ACIP. It was not indicated by the physician that child #1 is on a delayed schedule, and there was not an exemption note from the physician or the parent on file.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. The statement shall be kept in the child's record.

Noncompliance Area: During renewal inspection on 7/30/24, the file for staff person #1 included TB testing results that were more than 12 months old at the time of hire. TB test result date listed as 9/6/16.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: During renewal inspection on 7/30/24, the following physical site items were observed: visibly dirty and dusty ceiling vents were observed in several rooms through the facility. The plastic molding on the corner of the sink in the toddler B room was cracked and broken, leaving a sharp edge accessible to children. Several walls in rooms 2A, and 2B, and the door in the 3B room were observed to be visibly dirty.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: During complaint investigation on 6/7/24, due to self-reported non-compliance, it was determined that staff person #1 was physically aggressive with child #1 on 5/31/24, when pushing the child and his chair into the table. The harsh actions of staff person #1 resulted in child #1 pinching his/her thumb in between the table and the chair, causing the child to have a red and swollen thumb.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child.

This led to an incident between the two boys who were not properly supervised.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.
