Corrective Action: Ensure children are always supervised while in care. On July 8, 2020, a one-year-old child, left the indoor playground through a stair well door. Upon review of the video footage, the child was being observed by the two staff in the classroom until another child fell, drawing the attention of the staff members. The child was not supervised from 9:27 am until 9:31 am. The child did not exit the building. This was corrected when the staff members located the child in the stairwell. The parent of the child was informed within one hour of the incident. According to attendance records for the Sea Otters classroom, Staff #5, an aide without a completed comprehensive background check or the required qualifications to work alone with children, was left alone in the classroom each evening for the week of July 6, 2020, to July 14, 2020.
