Noncompliance Area: The infant room lacked copy of the emergency contact form for the young toddler who was receiving care in that room on the date of the inspection.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Noncompliance Area: Staff file #1 contained health assessments dated 10/13/17 and 10/29/19 which exceeded the 24 months required.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: Staff file #1 contained documentation of fire safety training completed on 1/19/19 and 5/14/20, which exceeded the requirement of annual training.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: The first aid kit in the young toddler room lacked sterile gauze pads in the kit.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: The toddler outdoor play space had loose clear tape at the seams of the outdoor carpet that posed a tripping hazard for the toddlers in care while playing in the space.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Child file #4 lacked the health insurance policy number on the emergency contact form.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: Child file #4, DOE 4/3/19, lacked an initial health report received within 60 days of enrollment at the facility.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: Child file #2 lacked an updated health report for a preschool child every 12 months as required.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: Child files #1, #2 and #3, all preschool age children, lacked documentation of immunization schedules updated in accordance the recommendations of the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The facility's emergency plan lacked a floor plan with designated shelter in place at the facility and evacuation routes to shelter location away from the facility.

Correction Required: The facility shall have an emergency plan that provides for shelter of children during an emergency including shelter in place at the facility and shelter at locations away from the facility premises.The facility shall have an emergency plan that provides for evacuation of children from the facility building and evacuation of children to a location away from the facility premises. The evacuation routes and evacuation plans to exit the building may be the same as those required by 3270.94 (f) and (g) (relating to fire drills).

Noncompliance Area: Child file #1 lacked parental consent for minor first aid on the emergency contact form.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care.

Noncompliance Area: There was Tylenol at the facility labeled for intended use by two children in care.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container.

Noncompliance Area: Staff file #1 contained valid health assessments dated 11/20/15 and 1/15/18 more than 24 months elapsed between assessments.

Noncompliance Area: There was a spray bottle with staff identified as sanitizing solution in the toddler bathroom that was not labeled with it's specific content.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: The hot water temperature in the children's sink in the Preschool room measured 125.5 degrees F.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.
