Staff #5 had documentation of mandated reporter training completed on 4/13/20 with previous training completed on 1/19/15. Staff #7 had documentation of mandated reporter training completed on 3/3/20 with previous training completed on 1/17/15. The file of Staff #6 lacked documentation of he National Sex Offender Registry Verification Certificate by July 1, 2020 (SEE LIS Code Sheet).

On 9/14/22 observed approximately two 1 inch holes under the 1st window at the side of the Indoor Play Space. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: Observed in the pre school room a wooden doll house with a loose screw at the top side of the roof; also observed a cracked plastic bin housing Legos.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: Observed in the infant room a crib not labeled for child #2 who was in the crib at the time of the inspection.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: Staff #1 was not able to name child #1 during the assessment of supervision at approximately 9:30 a.m. in the pre school room #111. At 12:55 p.m. staff #3 was not able to name 5 old toddlers assigned to her. At 1:00 p.m. staff #4 was not able to name 5 young toddlers assigned to her.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: The file of staff #2 with a hire date of June 24, 2015 had a Child Abuse clearance dated 7-8-15 and an FBI clearance dated 7-23-15.

Noncompliance Area: Plastic bags were located in unlocked cabinets under the sinks in the Infant Room and the Toddler Room.

Noncompliance Area: In the Toddler Room there was a refill bottle of Airwick Sanitizer, labeled Keep Out of Reach of Children, was located in an unlocked drawer and accessible to the children.

Noncompliance Area: Three Mums, a plant known to be toxic to children, were located on a window sill accessible to the children in the Toddler Room.
