Noncompliance Area: In the young toddler room, it was observed that the purple and green climbing mat was torn along the edges exposing the children to the foam inside that could brake off in small pieces.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: The file for child #1 lacked an agreement signed by the parent. The file for child # 6 lacked an agreement signed by the operator.

Correction Required: An agreement shall be signed by the operator and the parent.

Noncompliance Area: The file for child # 1 and #2 contained an original agreement form.

Correction Required: A parent shall receive the original agreement. The facility shall retain a copy of the agreement.

Noncompliance Area: The file for child #5, with a start date of 9/17/19, lacked an initial health report.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: The files for child #2, #3, #4, and #5 lacked an immunization record in accordance to the recommendations of the ACIP for the influenzas vaccination.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: The file for staff person #1 had state police clearance dated 8/26/14 and 1/21/20, child abuse clearance 9/11/14 and 12/17/19, FBI finger print clearance 9/3/14 and 12/11/19, and mandated reporter training on 12/19/14 and 1/18/29, all of which were completed after the 60 months. The file for staff person #2, with a hire date of 12/2/19, lacked National Sex Offender Registry clearance. The file for staff person #3, with a hire date of 12/4/19, contained a Department of Education finger print clearance and no proof of applying for the DHS clearance. It also lacked the National Sex Offender Registry clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 2 and #3-- may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #2 and #3--- may not work in a child care position at the facility.

Noncompliance Area: In the hallway outside of the infant room, hand sanitizer was hanging on a backpack that was accessible to the children.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: In the boys bathroom, no hand washing sign was posted above the urinal.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: There was only 3" of loose fill under the climbing apparatus to the left of the entrance way into the large play yard.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: The agreement did not specify the date of admission of child #3

Correction Required: An agreement shall specify the amount of the fee to be charged per day or per week.

Noncompliance Area: The Emergency Contact form for child #2 did not have on it the complete name, address and phone number of the child's physician

Correction Required: Emergency contact information must include the name, address and telephone number of the child's physician or source of medical care.

Noncompliance Area: Children #1 through #4 did not have up to date immunization's.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP.

Noncompliance Area: in the 2's room, there was Tylenol in a box with the child #4's name on it but no medication log was written out to accompany it.

Correction Required: An operator is responsible to establish and maintain a medication log if prescription or nonprescription medication is administered.

Noncompliance Area: In the pre-school room, there was a box of Equate pain reliever with child #5's name on it. On the medication log, there was there was no dosage amount written in.

Correction Required: A medication log shall include the amount of medication administered.
