Upon arrival, 5 children present with 2 caregivers. Ms. Sullivan let me in the front door and the other caregiver was inside changing the infant's diaper. Upon arrival, both caregivers were inside the home, 2 children were in the doorway and two children were outside in the fenced play area alone.

Provider had just given a new staff member diaper changing instructions on playground in back of house, when state representative knocked loudly on the front door and provider went to open door to let the state representative in they headed towards the hallway which leads to the playground, representative ask if new staff member was present and provider stated "yes on the playground with other children" representative notice that the new staff member had came in and was changing an infant while 2 children stood in the door and the other 2 children were sitting at the picnic table.
