Findings: The staff members state that they had a group of 9 children outside on the playground at the time. One of them was on the blacktop with the bikes, and the other was standing near the play structure. The child was observed climbing the slide. The staff member told the child not to do that, but when the child turned to look back at them they walked over the right side of the slide and fell on their left arm.
