Noncompliance Area: School aged children were interviewed individually and they report that they have seen facility person #1 pull children by the ear and grab children by the arm.

Correction Required: A facility person may not use any form of physical punishment, including spanking a child. The legal entity must arrange for all facility staff to receive a minimum of two hours of PQAS approved training regarding appropriate discipline/behavior management. The legal entity must receive DHS approval of the training content prior to scheduling the training. This training may be online or in-person but may not be completed while children are in care. .

Noncompliance Area: Interviews were conducted with several school aged children separately. During these interviews several children reported that facility person #1 allows children to nap in the basement unsupervised while she is upstairs and also facility person #1 also uses the computer in the kitchen while children are in the living room watching TV. The living room is not visible from the computer desk.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in 3290.51-3290.52 (relating to maximum number of children; and staff:child ratio). Children must be supervised at all times. Noncompliance Area: Several school aged children were interviewed individually and several report that facility person #1 yells loudly and uses harsh language with and in front the children causing some children to be frightened.

Correction Required: A facility person may not use harsh, demeaning or abusive language in the presence of children.

Noncompliance Area: On 3/12/2020 during a complaint investigation, Rep observed 8 unrelated children in care at one time. One related child was also present. When 6 school age children left for school, the number of unrelated children present was fewer than 6.

Correction Required: The number of children in care may not exceed six children at any one time who are unrelated to the operator.

Noncompliance Area: Child #1 has a fee agreement and emergency information form that had not been updated since 1/18/18.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.
