Findings: Complaint: The provider called this specialist to state that her grandchildren were last in care on 12.3.20 after testing positive for COVID 19 on 12.4.20. Despite the providers grandchildren testing positive for COVID-19, she did not reach out to this specialist until 12.14.20, but only after the Health Departments (HD) Contact Tracing Unit reached out to her then directed her to do so. The provider stated she closed immediately after finding out about their results but admitted to reopening last week, despite not reaching out to the health department for confirmation or approval first. Complaint confirmed. The complaint was self reported. However, the provider clarified that said she never closed after finding out her grandchildren tested positive for COVID 19 but that she did notify the other 4 parents about the incident, which resulted in two of kids coming back to care while the other two remained home. The provider called the 4 parents to forewarn them about the incident but not OCC and HD. Provider informed the licensing agency should always be notified immediately of any changes in residents, operation, contact number's and the residence. See remarks section.

The PNP shall only contain the child, the fitted sheet and thin blanket (if necessary).
