|Findings: Based on staff interviews and documentation observed, the surveyor determined that the licensee or a staff member did not report suspected or alleged child abuse or neglect per rule requirement. Cross-reference: ARS 13-3620 which is noted in R9-5-307.1 During interviews with multiple staff members, the surveyor was informed of possible abuse or neglect to an enrolled child, at a location other than the facility. The child has been identified as Child #2. Documentation submitted shows that on 5/9/17, two staff members discussed knowledge of the child exhibiting the following behaviors: Child showing private body parts to another enrolled child; child exhibiting "clinginess," and child frequently wetting self while awake. Documentation shows suspicion of abuse was discussed. Staff #1, Staff #2 and Staff #3 acknowledged having suspicion of abuse. Staff #1, #2 and #3 acknowledged that the Department of Child Safety (DCS) had not been contacted upon suspicion. Staff #1 stated having contacted DCS on 5/18/17.|

|Findings: The surveyor reviewed 2 staff files and observed the following: Staff #1 did not complete 18 annual hours of training during her first 12 months of employment at the facility.|
|Findings: Per allegation: Infant room ...the surveyor entered the infant room directly behind Staff #5 who was entering the infant room from the hallway; a staff member had not been present with the two enrolled infants in the infant room while Staff #5 was in the hallway leaving the ratio as 0 staff members to 2 infants. Staff #5 stated that she was assisting a new staff member with sign in issues on the computer. This rule was previously cited on 1/4/17.|
|Findings: The surveyor entered the facility and then observed the following: Staff #5 in the facility hallway; Staff #5 stated that she was assisting a new staff member with sign-in issues relating to the computer. Staff #5 was not listed as a designated person in charge but was performing facility administration - in doing so, the responsibility took her from her classroom as an infant teacher - leaving the infant room out-of-ratio. Staff #1 was listed as the staff member in charge when the director and director's designee are absent from the facility. Staff #1 was the only staff member in her classroom and could not perform administrative responsibilities to the staff member needing assistance with the computer.|
|Findings: Per allegation: Infant room ...upon entering the infant room behind Staff #5, the surveyor observed an infant crawling out from under the crib located closest to the door being entered; there was a lack of supervision when there was no staff member in the infant room with the two enrolled infants and when one infant had been under a crib for an undetermined amount of time.|
|Findings: The surveyor observed the following upon entering the classroom shortly after 11:53 AM to count ratios: School-age classroom...enrolled children were watching a movie on video. When the surveyor entered the room again at 1:24 PM to interview the staff member present, the movie had just finished and a second movie was being picked out. Quiet time, reading, language resting and puzzles was noted on the lesson plan and schedule for that time frame, but was not being implemented.|
|Findings: The surveyor observed the following: School-age children were in a room used for enrolled 2-year-old children; the room had an approved diapering area, high chairs and toddler equipment. Age appropriate toys, material, and equipment were not provided for the enrolled school-age children present.|
|Findings: The surveyor observed the following: Enrolled children eating Ritz crackers and water for a snack; two food components were not served at snack. Staff #2 and #4 acknowledged the lack of a second food component.|
