Two children who, on 10/10/2023, provided a Medical Statement for Meal Modification that included changes to include cooked milk instead of just liquid (fluid) milk was not updated in either the kitchen or the classroom where the children were eating.

When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. In Classroom #3 the children with NC PreK children there was not a cumulative log reporting screen time. When asked for the log the teacher was able to produce last weeks' screen time log.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In classroom #1 that cares for infants there was a large plastic bag accessible to the children.

Developmentally appropriate equipment and materials were not provided for variety of outdoor activities for vigorous play and large muscle development. On the playground there is one structure and no other equipment or materials out and available for the children to play with. This has been an ongoing issue and there is a cabinet with some additional materials in it, but it is not opened or made accessible to the children in care. This is a repeat violation.

When three-year-old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. In space 6 there was only books in the Language area. There were no other materials for the children to be able to use to have the three activities. This is a repeat violation.

In Space 5 where the two-year-old children were located there was a minimal number of materials available.
