All three children had " Northwest Hospital " listed as a contact. The Director was advised that a hospital is not considered an emergency contact for a child.

Findings: Surveyor 2 reviewed 10 children ' s emergency cards and the attached immunization records and observed the following: * Child 4 was missing a copy of their immunization record.

Findings: Surveyor 2 reviewed the sign-in and out logs for January 2022 and observed the following: * Child 5 was not signed-out on 01/21. * Child 6 was not signed-out on 01/14. * Child 7 was not signed-out on 01/20. * Child 8 was not signed-out on 01/17. * Child 9 was being signed-in and out with initials only all month.

Findings: * During the inspection of room #19 (pre-k), Surveyor 1 counted 11 children physically present. The roster had 10 children marked as present. * During the inspection of room #4 (three's), Surveyor 1 counted seven children physically present. The roster had eight children marked as present. * Surveyor 1 counted three children in the tent building attending a physical education class. The staff member acknowledged that she did not have a roster.

Findings: During the inspection of the diaper changing area in room #10 and room #15, Surveyor 1 observed that both of the diaper changing surfaces had bolts in the side walls that had a piece of vinyl strapping attached to them. The straps are not sanitzable.

Findings: During the inspection of the playground area, Surveyor 1 observed that the dirt had been washed away from under the gate at the far west side of the playground, leaving a hole that was 11 inches deep.

Findings: Surveyor #2 reviewed the sign in and out records and found the following: 1. Child #1 was not signed out on 1/11/21. 2. Child #2 was not signed out on 1/5/21 and 1/11/21. 3. Child #3 was signed out with only a first name on 1/5/21.

Findings: Surveyor #1 observed that there were 5 children in the ones and twos classroom but 6 children on the roster. This was corrected at the time of inspection.

Findings: Surveyor #2 observed that the gas inspection was dated 11/29/19 and not within the last 12 months. A copy of the current gas inspection was sent to Surveyor dated 1/15/21.
