Findings: Surveyor 1 observed nine children from one of the four year old classrooms on the playground with their teacher-caregiver and requested to view the classroom roster. Findings: During the inspection, the Surveyors reviewed two staff files and determined that: -Staff #1's Fingerprint Clearance Card expired on 3/29/2016 and the following application was not received by DPS until 4/18/2016. Current Fingerprint Clearance Card was verified during inspection and is valid. -Staff #2's Fingerprint Clearance Card expired on 1/4/2018 and an application had not been received by DPS as of the inspection date of 9/5/2018.
