The Notice of Inspection Rights was provided during the inspection. Please submit the Plan of Corrections through the Licensing Portal within 10 days of receipt of the Statement of Deficiencies. Licensure is pending corrections of all deficiencies. Two of two Fingerprint Clearance Cards were verified as valid through the DPS website.
