Findings: On 7/19/19 at approximately 9:00 AM, the Surveyors arrived at the facility to conduct the annual compliance inspection. They rang the doorbell for approximately 5 minutes and called the facility twice. It appeared that no one was in the lobby but the Surveyors were unable to determine if anyone was present inside the main classroom area. Surveyor #1 left a business card and the Surveyors departed. The Director subsequently emailed Surveyor #1, stating they were closed that week to get ready for the upcoming school year, but that they had also been closed on Fridays all summer. The Surveyor informed the Director that written notification is necessary for such changes and the Director understood. (Reference: R9-5-208.B.7.a.iii.)
