Additionally, there was a drying pad and a plastic bin on the diapering sink counter in the (older) Infants diaper changing area.

Findings: Based on review of staff files, as well as documentation from the DPS website, the Team Leader noted that Staff 2 did not apply for a Fingerprint Clearance Card within seven days of the documented start date of employment. Staff 2's start date of employment is listed as 10/28/2019, but DPS did not receive Staff 2's Fingerprint application until 11/12/2019, and their fingerprint card was issued 11/15/2019.

Staff 2's TB test results are dated 10/30/2019, and their start date is documented as 10/28/2019.
