Initial Comments: The following deficiencies were observed at the Compliance Inspection conducted on 4/15/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. The following was discussed but not limited to: 1) New rules effective 8/2025, 2) Removal of the gate that encloses the diapering area in Rooms 8, 9 and 10, 3) Tummy time rules, 4) Revising capacities in infant rooms 7 and 8, due to the reduction of the number of cribs in both rooms and the related modification application. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.
INSP-0124236 2025-04-16 Compliance (Annual) Complete
Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 4/16/2025 and are subject to changes pending programmatic review. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1) Licensee informed of the DCS portal that was effective in August 2024 and all providers are required to submit DCS clearance for all staff through this portal, 2) Tummy time rules and ensuring the firmness of tummy time mats are maintained, 3) Date of employment (versus start date) as it relates to staff obtaining fingerprint clearance cards, licensee verification of fingerprint clearance cards through DPS, orientation training completion, obtaining TB test etc, 4) Relocating the soiled clothing container that is located on a shelf that is near the food prep sink, and 5) Photos of the 5 additional licensed cribs that are currently in the basement storage, since the storage area was not accessible at the time of the inspection. Please submit documentation of foreign degree equivalency for Staff #1, #2 and #4, when obtained. There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 05/01/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was completed at the time of the inspection. The DES Contact Group Size was in compliance at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *The bases of the exterior fencing is beginning to rust. Compliance Officer is Sherri Pavlisick.

Initial Comments: The following was observed at the time of the Modification inspection conducted on 1/30/2024, and are subject to change pending programmatic review. A full on-site inspection was not conducted at this time. Please submit the Plan of Corrections via the LMS portal within 10 days. The following was discussed but not limited to: 1. The fees for the license. 2. Rules under review. 3. Only infants are allowed in the infant room when 6 or more children are present at the facility. Compliance Officer is Tricia Tartaglio

I only gave this place one star because 0 wasn't an option. Do not take your children here!!! This place is filthy and not up to DHS standards! Staff is undertrained and impatient. The director Rhonda is flighty and a total space cadet! She lets her own children wander around the center all day long and nobody says anything because shes the bosses kid! The other director Heather is rude to clients, kids and employees. She has been overheard talking bad about parents of children and she has even called her own child a "little a$$hole" on campus! and she and the other director, Rhonda have had screaming matches behind closed doors! Totally unprofessional, filthy and not worth the ridiculous price to bring your kids there! Most of the teachers there don't even like kids and the ones they do like get spoiled and extra attention that all kids should be getting. This place should be closed down and seriously inspected by the state!!
