Staff #1 was not qualified as a teacher caregiver because the file lacked documentation of a high school/ high school equivalency diploma. Staff stated Staff #1 sometimes works alone with enrolled children.

Staff #2 (DOE 10/2/14): 7 of the required 18 hours of annual training from 10/19 - 10/2020. Staff #3 (DOE 10/1/18): 2 of the required 18 hours of annual training from 10/19 - 10/2020. Staff #4 (DOE 8/6/18): 12 of the required 18 hours of annual training from 8/19 - 8/2020. Staff #5 (DOE 3/21/05): 17 of the required 18 hours of annual training from 3/2020 - 3/2021.

Findings: Based on the Surveyors' observations in the outdoor activity areas and staff interview, the following hazards were accessible to enrolled children: North playground: * There was warped and splintered wood on the bottom section of the shed. * There was corrosion, rust and peeling paint along the south and east sections of the wrought-iron fence. Southwest playground: * There was corrosion, rust, and peeling paint on the bottom of the middle shade post. * There was an uneven surface (about 10 inches wide) in the Astro-turf surface, posing a tripping hazard to children. A staff stated they filled it with sand but it was moved away by the children. Southeast playground: * There was rust on the bottom of the fence.

Findings: Based on Surveyor & Team Leader's observations of 2 playgrounds, the following was observed: South playground lacked: * 5 of the required 6 inches of wood chips in the fall zones of the steps on the climber. * 6 inches of resilient surface in the fall zones of the round, red tunnel with 2 black steering wheels. North playground: * 3 inches of dirt/wood chips mix in the fall zones of the wall climber.

Findings: Based on the Surveyor & Team Leader's observations in the outdoor activity areas, the following hazards were accessible to enrolled children: South playground: * There was a broken hinge on a cabinet near an exterior building. * There were 9 areas of exposed rebar with rust, and 2 sharp metal edges on metal pieces in the garden boxes, and there was exposed landscape fabric inside the boxes. * There was broken, missing, and cracked stucco on the lower section of the storage area, and the inner mesh wiring under the stucco was exposed. * There was a large area of rust on 3 of the 4 shade structure posts. * There was corrosion, rust, and peeling paint on the bottom of the southeast shade post. * There was an uneven surface (about 10 inches wide) on the Astro-turf and the turf was not secured to the ground. A staff stated a tree had been removed. * There was rust along the bottom section of the wrought-iron fence posts. * There were 3 sections of uneven sidewalk near the east side of the playground. North playground: * There were rusted areas near the tops and the bottoms of the fence posts. * There was peeling paint inside the red playhouse and on the red stove. * There were large sections of Aloe Vera hanging in the wooly baskets on the north wall.

I only gave this place one star because 0 wasn't an option. Do not take your children here!!! This place is filthy and not up to DHS standards! Staff is undertrained and impatient. The director Rhonda is flighty and a total space cadet! She lets her own children wander around the center all day long and nobody says anything because shes the bosses kid! The other director Heather is rude to clients, kids and employees. She has been overheard talking bad about parents of children and she has even called her own child a "little a$$hole" on campus! and she and the other director, Rhonda have had screaming matches behind closed doors! Totally unprofessional, filthy and not worth the ridiculous price to bring your kids there! Most of the teachers there don't even like kids and the ones they do like get spoiled and extra attention that all kids should be getting. This place should be closed down and seriously inspected by the state!!
