Findings: Surveyor 1 requested the attendance rosters for both activity areas. The Staff had not completed an attendance roster. The Staff were unaware of how many children were present. Staff 1 thought there were 52 children present, however the Surveyor and Staff counted 38.

Findings: Surveyor 1 and 2 observed the following ratio violations: * Surveyor 2 observed a child alone on the playground with no staff to meet the staff to child ratio. * Surveyor 1 observed the children go back and forth to the restroom and change classrooms through a bathroom and hall with no staff to maintain ratio.

Findings: Surveyor 1 and 2 observed that the staff were completely unaware and unaccountable for the children in their care. An adult called to have a child get ready. The Staff were not aware of who the child was that the adult was calling about, and whether the child was even at the facility. The staff were not aware that the child left, nor who they left with. Surveyor 1 observed a large amount of water on the floor of the boys bathroom and the cafeteria creating a fall hazard. Surveyor 1 and 2 observed two licensed programs sharing the cafeteria activity space. Enrolled and non-enrolled children may not share the same space.

Findings: The Surveyor observed mops, a bucket and vacuums in the cafeteria accessible to the enrolled children.

Findings: Surveyor 1 and 2 observed that the staff did not supervise the enrolled children. Staff 1 did not supervise the children during transitioning from the cafeteria, through a hall, through a bathroom and then into the other activity area. Staff 2 and another staff member in the room were not supervising Child 3 who went out into the parking lot and to the playground without any staff to supervise.
