To meet the meat alternative requirement, each 1 to 2 year old child should be served one sandwich and each 3 to 5 year child should be served one and one-half Uncrustable Peanut Butter and Jelly sandwiches. One Uncrustable Peanut Butter and Jelly sandwich was served to each 1 to 5 year old child as a meat alternative and grain requirement. Potato chips of any kind do not meet the requirement for a vegetable. Baked Lay's potato chips were served to each child 1 to 5 years of age as a vegetable component. Serving one Uncrustable sandwich to children 3 to 5 years of age as a meat alternative and serving Baked Lay's potato chips to children 1 to 5 years of age as a vegetable component does not meet the requirements for Meal Patterns for Children in Child Care Programs from the United States Department of Agriculture (USDA).

One medication permission to administer form authorizing permission to administer a prescription emergency medication had expired in July 2023 and the medication which had not expired and had not been used was not returned to the parent within 72 hours of the expired medication permission to administer form. The interim director notified the parent by phone during the visit. The parent picked up the medication from the office, brought a new medication, and completed a new permission to administer form during the visit.

Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Staff medical records for two staff members with dates of employment of 08/30/2021 and 05/15/2023 did not include a staff health assessment/medical report that included a statement signed by a health care professional that indicated the person was emotionally and physically fit to care for children.

Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. In one child record file, the parent Discipline Policy acknowledgement statement did not include the date of enrollment for the child.

The signed Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy staff acknowledgement statement for one staff member with a date of employment of 05/15/2023 was dated 05/26/2023.
