Findings: Per the allegation, it was determined from observation and interviews with children that children are instructed to nap on their stomachs with their arms at their side or underneath their face. Surveyor 1 observed more than 3/4 of the children in the preschool and school age rooms napping on their stomachs with their hands at their sides on the day of the inspection. Based on staff interviews, children are required to nap and are not given an optional activity when they do not want to nap.

Findings: Upon inspection of the facility's attendance records, the Team Leader and Surveyor 1 noted the following: -there were 11 children present in the outdoor activity area, however the roster listed 12 children as being present; -there were 12 children present in the school age activity area when the Team Leader entered the area. The facility director reported that there was no roster in the room.
