Description: The mission of Apple Tree Academies and Affiliates is to provide professional, consistent, safe, secure and high quality childcare to all the children and their families that attend our centers. We have educated children for over 30 years and operated centers throughout the southeast. Currently, we operate in North Carolina and Virginia. Most ATA centers accept children from six (6) weeks to ten (10) years of age, with certain locations serving children up to age twelve (12). A safe indoor and outdoor environment was not provided for the children. During departure time, I observed parents pulling under the awning and parking their cars while they entered the center to pick up their child. During the same timeframe, public school buses arrived and parked just outside the awning. I observed children departing the bus and walking across the area covered by the awning (where in some instances cars were parked - one pickup truck was left running. The director stood in the doorway to meet the children as they got off the bus. The children getting off the bus in an area where cars are parked and/ or left running is not a safe practice.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Two gallon size buckets of paint were stored in an unlocked classroom. Albuterol was stored in the director's office on a low shelf by the doorway while the office was left open and unattended.

A safe indoor and outdoor environment was not provided for the children. On the playground used by infants and toddlers, the swing brackets and chains connected to the two swings for infants were observed to be rusted.

In Space 6, alcohol swabs, antiseptic cream, and an ice pack labeled Keep Out of Reach of Children with additional warnings was observed in a First Aid kit hanging greater than five feet from the ground, but in unlocked storage.

Parent's medication authorization did not include required information. In Space 3, the medication authorization form for a topical ointment was not signed by a parent for one child enrolled.

When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. In Space 1, a medication log was not on file for two children enrolled with a prescription diaper cream.

The EPR Plan did not include the location of the Ready to Go File and or the required information. The Ready to Go File was missing the medical action plan and medication authorization for one child enrolled with a food allergy.
