A child was restrained as a form of discipline and the child's safety or the safety of others was not at risk. On January 2, 2024, the staff member appointed as Director restrained a three-year-old child using her leg to put the child's shoes on while the child was crying and screaming. This staff member then used her arm to restrain the child to put duct tape around the child's shoe, sock, and bottom portion of the pant leg on both feet and legs individually while the child was crying and screaming.

In space #3, one electric fan powered on and in use was observed on a shelf higher than five feet from the floor and a second electric fan that was not in use was observed on top of a cabinet above five feet from the floor and both electric fans were not mounted.
