Findings: Upon arrival at the facility, the Team Leader and Surveyor observed the Director open the front door and step outside. Once inside the facility, the Director acknowledged that she had left the children in the room (room #4) unsupervised. The Team Leader counted 10 children physically present in the room. The Director was observed stepping out of the room a number of times.

Findings: Upon entrance to the facility, the Team Leader and Surveyor observed that there were not enough staff members to operate the facility as there was not extra staff to use for breaks, to serve lunch, or answer the phone. The Director was observed on the phone on more than one occasion while supervising children in room #4. There were 19 children on the playground and while the two staff members were in ratio, there were not enough staff present to bring all of the children into the building due to room size capacities.

Findings: The Surveyor observed that the mats found in the hallway closet area were stacked directly on top of each other. The mats had sheets on them and they were all in direct contact with the next mat, not leaving any space or barrier between them for sanitation purposes.

Findings: During the inspection of the diaper changing area in room #1, the Surveyor observed that there was a tube of diaper rash ointment that was not labeled with a name on it. The ointment was in the cubby labeled with the name of Child 10.
