Violation Observed: Based on information, ensure that adequate supervision is maintained at all times throughout the center, including during naptime, on the playground and during drop off and pick up times. A named child left the playground unsupervised and was found in the road by a passing motorist. Additionally, a named staff was using her cell phone while caring for the children and the staff would leave the children unsupervised while answering the door's during drop off and pick up times. On 6/22/2022, a child was left unsupervised in the bathroom.
