Initial Comments: Based on Surveyor's and Team Leader's review of staff #2's file (date of employment 10/25/2021), it was determined that the staff was issued a fingerprint clearance card on 12/7/2021 and the staff did not have a valid fingerprint clearance card upon employment.

Initial Comments: Based on a review of staff #1's file (date of employment 8/2/2021), the Surveyor and the Team Leader determined that the file contained documentation of the staff's freedom from infectious active tuberculosis that was completed on 11/8/2021, rather than on or before the staff's starting date of employment; and Based on a review of staff #3's file, the file lacked documentation of the staff's freedom from infectious active tuberculosis on or before the staff's starting date of employment. NOTE: This rule was also cited at the time of the 5/2021 and the 10/2021 inspections.

Initial Comments: Based on Surveyor's and Team Leader's observations, 13 children were present in the Purple room but the roster had the names of only 6 children listed as present. Eleven children were present in the Blue room, but the roster had the names of 12 children listed as present. The staff stated a child had left for the day and the child had not been marked off the roster. NOTE: This rule was also cited at the time of the 5/2021 and the 10/2021 inspections.

Initial Comments: Based on Surveyor's, the Team Leader's and the Licensee's observations, the following unrelated items were found stored on the diapering counter: *Purple room- a plastic finger paint tray was stored near the diapering sink by the sink faucet. Dried paint was observed in the diapering sink drain. The staff acknowledged that she had washed the paint tray in the diapering sink; and * Orange room- A wet rag, an orange paint container and a sponge was stored on the diapering counter. NOTE: This rule was also cited at the time of the 9/2020, the 5/2021 and the 10/2021 inspections.
