| Initial Comments: The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 6/30/2025, and are subject to changes pending programmatic review. A full inspection was conducted at this time, with the exception of staff files. The Director's and one other staff members files will be emailed to the Compliance Officer for review prior to starting with enrolled children. Fingerprint cards will be verified on the files submitted through the DPS website. Please complete the Plan of Corrections via the LMS Portal within 10 days of receipt of this Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Emergency Disaster Contact Form was completed at the time of the inspection.
