INSP-0164191 2025-12-02 Complaint Complete
Initial Comments: The purpose of the inspection was to conduct a Complaint investigation #150771 on 12/02/2025. A full inspection was not conducted at this time. The Plan of Corrections will not be accepted at this time. A paper copy of the Notice of Inspection Rights was given to the Facility Director at the beginning of the inspection. Ratios observed were: Infants: 2:4 1s-3s: 2:7, 1:4, 2:10, 1:4 3s-6s: 2:19, 1:10 Documentation observed: classroom rosters, diaper logs, and 2 staff files There were 6 staff interviewed during this investigation The fingerprint clearance cards for 1 of 2 staff members were verified to be valid, and 1 of 2 was verified to be not valid through the DPS website at the time of the inspection. Upon completion of the complaint investigation #150771 it was determined from observation, interview, and documentation, that 1 of 2 allegations were substantiated and 1 of 2 allegations were unable to be substantiated due to a lack of sufficient evidence. Initial Comments: The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 9/29/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the facility at the time of inspection. The fingerprint clearance cards for 26 of 26 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following was discussed, but not limited to: *Staff files will be updated when the license is released: New Start Date, Fingerprint Verification, DCS Central Registry, TB Attestation, and new 10-day training *Staff who are alone with children will meet the qualification of Child Educator *Medication authorization forms will include the reason for the medication *The ratio of staff members to resting infants in the resting area (floor beds) must be at least one staff member to every four infants. Any staff member in the resting area shall have current certification in CPR/First Aid. *The diaper handwashing sink will be within one step or pivot of the diaper changing surface *The water temperature at the diaper handwashing sink will be maintained between 86-110 degrees *Continue to monitor the outdoor equipment to ensure there is no splintery wood accessible to enrolled children *Toilet seat inserts will be maintained in a clean condition *Children 2 years and older may use the gross motor room
