In outdoor space 1, the rubber coating was peeling causing a tripping hazard and exposing rust, the basketball goal had peeling paint and exposed rust, one corner of the white playhouse was coming apart, and the blue castle had sharp edges. In space 3, there was white out in a desk drawer.

Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. In space 1, there was albuterol with a prescription label adhered to it, but it was not in the original prescription box.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space 1, there were two (2) tubes of expired Butt Paste with expiration dates of January and March 2026.

A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The Administrator did not notify the Division within five (5) of an employee that was hired on hired on January 12, 2026.

A safe indoor and outdoor environment was not provided for the children. The black border surrounding the sand box under the shade port was cracked creating opening for children’s hands to get stuck on playground #1. A green toy pail was cracked exposing rough edges, and one bolt was protruding from the black border on playground #2. Two containers of whiteout were observed in an unlocked drawer inside the teacher’s desk that is accessible to children.
