A safe indoor and outdoor environment was not provided for the children. On the school age playground the covering of an utility box was missing exposing wires and on the two year old playground staples in the shed doors were protruding.

The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Tissue and debris were located on the playgrounds. There were broken cones, creating sharp edges on the two year old playground and a broken sifting toy on the preschool playground. The broken items were discarded during the visit.

In prekindergarten spaces #1, 2,3,4 and laminators were stored below five feet. In prekindergarten space #2 batteries, and a hot glue gun were stored in cabinet drawers below five feet.

Staff did not wash their hands thoroughly before beginning work, before/after handling food, before bottle feeding or serving to other children, after toileting or handling body fluids, after diaper changing and after handling soiled items. In space 4 a teacher did not follow the hand washing steps for diapering, staff did not clean their hands after removing soiled diaper.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the staff bathroom a can of Ozium Air Sanitizer was on the counter below five feet. In the entry to the buildings hand sanitizer was stored on shelfs below five feet. In space one hand sanitizer was stored in the emergency bag on top of a shelf below five feet. In space four stored in a cabinet above five feet but not locked was pepper spray attached to keys.

A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space four a container of petroleum jelly did not have an authorization form.

Parent's medication authorization did not include required information. In space three authorization for a tube of A&D ointment did not have the amount to be applied. In space five authorization for Coppertone kids sunscreen did not have the name of the sunscreen listed nor the amount to be applied and authorization for an asthma inhaler did not include the route to be administered, the symptoms of when to be administered, nor an end date for the authorization form.
