Violation Observed: Ensure that materials are accessible to the children at all times, except during lunch and nap time in room 5. Room 5 needs to have 5 activity areas with 5 supplies in each area. Supplies were also being stored on the window ledges and up high where children could not reach them.

Violation Observed: Based on complaint # 21913, designate a director designee for whenever the director is not present at the center and ensure that the director designee does not have full-time classroom responsibilities during the times the director is not at the center. There is not a director designee at the center at all times from 7am to 9am when the director is not present, and at the time of inspection the director designee was needed for ratios in room 1. .

Violation Observed: Ensure that the center maintains daily time sheets for the children that indicate accurately the children's departure times. Children who are picked up by the school bus are being signed out after they leave the center and not the time the bus picks them up.

Violation Observed: Ensure that all playground equipment meets public playground design standards (ASTM F-1487). Repair the crack in the stairs on large climber and ensure the bolts on the smaller climber are free of rust. 3-8-23: The center ordered new stairs however is waiting for installation. 3-23-23: The center is still waiting for installation.
