Findings: There are children who are missing the complete health inventory or part of the health inventory as well as documentation of immunizations. The center was advised as to which children need the documents added to their file. The parent must complete part I of the health inventory and part II must be completed by the physician. The physician should attach a copy of the child's most current immunizations to the health inventory. There are also several emergency cards that are not complete (missing doctor information, parent signature). Some of the emergency cards printed do not have the parent signature portion at the bottom due to the way the form was printed. The center was advised of which children are missing documentation on the emergency form and will have parents complete the information.

Findings: Fifteen children need documentation of lead blood tests. These tests are to be done at 12 and 24 months of age. If a child only has one test completed and it was done prior to the child turning 2 years old, another lead blood test must be done. If the child only has one test completed but it was done after the child's 2nd birthday, then no more lead blood tests are required. Four of the fifteen children need documentation of the 2nd lead blood test. Many children had lead forms from MDH in their file where the parents signed an exemption because the child has not lived in any of the at risk zip codes. This does not apply to child care so the lead blood tests must be completed. A parent may religiously object to the lead blood testing and can sign in the appropriate box on page 4 of the health inventory (for a true religious exemption, if the child had the first test then the 2nd test should be done). Please have the parents obtain the lead blood test documentation from the physician and add to the children's files.

The program is trying to be proactive during the COVID pandemic and keep all surfaces as clean as possible. However, these sanitizing items must be kept inaccessible to children. The use of hand sanitizer with the children must be limited when soap/water hand washing is available due to the alcohol content in hand sanitizers. Hand sanitizers should not be used as a substitute for regular hand washing. The center must ensure these items are placed out of reach or inaccessible to the children in care.
