| Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. One child who was identified as having a milk intolerance and who was allergic to bee stings did not have a medical action plan attached to their application or on file today. All children who have a chronic medical condition must have a medical action plan completed and attached to their application. Please obtain the applicable Medical Action Plan from the child’s parent and attach a copy to the child’s application and retain it on file at the facility for future review. The child’s application stated an Epi-Pen would be provided in case of an emergency, but you said they have not brought one to be kept at the program. | |||
